Job summary

Kaiser Permanente in California's Palm Desert Medical Offices is seeking an RN with oncology experience to collaboratively manage patients' needs and develop tailored care plans. You will coordinate access to health services across multiple providers and disciplines, monitor outcomes, and support discharge planning as part of a multidisciplinary team.

Requirements include a California RN license, Basic Life Support, bilingual English/Spanish, and utilization review knowledge.

Qualifications

• Minimum two years clinical experience as an RN in an acute care or ambulatory care setting.
• Cancer Program: Two years clinical experience in Oncology.
• Bachelor's degree or equivalent experience four years required.
• Registered Nurse License (California) and Basic Life Support.
• OCN within one year of hire.
• Bilingual (English/Spanish) Level II required.
• Case Management Certification or certification in the area of specialty preferred.
• Bachelor's degree in nursing or healthcare related field preferred.

Responsibilities

• Evaluates and identifies members needs and interfaces with Primary Care Physicians, Specialists and various disciplines on the development of case management plans/programs.
• Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.
• Coordinates interdisciplinary approach to provide continuity of care including utilization management, transfer coordination, discharge planning and authorizations/transfers as needed for outside services.
