Fred Hutchinson Cancer Center in Seattle is seeking a Clinical Nurse Coordinator (CNC) to provide care management for a complex outpatient panel. You will serve as primary contact for patients and coordinate with an interdisciplinary team to ensure continuity of care and optimal outcomes.

Required RN with WA license, BLS and oncology background; training is provided for applicants without oncology experience.

Qualifications

• Graduate from an accredited school of nursing.
• RN license in WA or valid multistate license (MSL).
• Current BLS HCP card required.
• Oncology experience preferred; training will be provided for non-oncology applicants.

Responsabilités

• Assess patients' physical, emotional, social, and spiritual needs.
• Develops a care plan based on disease, symptoms and response to treatment.
• Educate patients and families on disease processes, treatments and side effects.
• Coordinate with inter-disciplinary team to ensure continuity of care.
• Direct patient care activities including education on chemotherapy and injections.
