Job summary

Medical University of South Carolina seeks an Oncology Patient Coordinator to support care coordination for oncology patients and act as the primary contact for new and existing referrals. Qualifications

• Bachelor degree from an accredited university and 1 year of healthcare experience or high school diploma and 5 years of job experience in referral management, oncology navigation or patient access.
• Strong interpersonal skills, ability to establish and maintain collaborative relationships both internally and externally, strong organizational skills.
• Ability to rapidly prioritize/reprioritize and communicate effectively in written and verbal form.

Responsibilities

• Assist the patient to better direct their own health care experience using screening and reinforcement of the treatment plan, educational programs, and linkage to health services and community resources.
• Serve as a patient liaison to assist patients and caregivers in scheduling within the complex healthcare system.
• Act as an advocate for an individual's health care needs by identifying and communicating opportunities for care intervention that have been established by the RN Navigator or Coordinator, including identifying and addressing gaps in care.
