Qualifications

Responsibilities

• Educate patients and families and coordinate with the health care team.
• Coordinate care and appointments across departments and services.
• Support multidisciplinary tumor board preparation and data collection for regulatory and operational reports.
• Lead and participate in care coordination, transitions, and documentation for patients.
