Job summary

Atlas Oncology Partners seeks an Advanced Practice Provider to deliver direct clinical care to cancer patients in collaboration with partner oncology practices. The role involves diagnosing, evaluating, and managing cancer-adjacent conditions and coordinating care with primary and specialty providers to optimize outcomes across the cancer journey.

Reporting to the Medical Director, you will participate in interdisciplinary care teams, contribute to quality measures, and support program delivery

Qualifications

• Current certification as a Nurse Practitioner or Physician Assistant.
• At least 4 years of direct patient care experience in primary care and/or oncology.
• Extensive knowledge in chronic disease management and palliative care.
• Strong ability to collaborate with diverse teams and foster a positive work environment.

Responsibilities

• Provide direct, evidence-based primary care to patients with cancer, office and virtual.
• Collaborate with oncology providers managing the patient’s cancer.
• Coordinate care and referrals to primary and specialty care.
• Participate in specialized clinical programs (CHF, frailty, behavioral health).
• Provide transitions of care visits and escalate issues when needed.
• Collaborate with leadership to improve clinical operations.
