Job summary

Atlas Oncology Partners seeks an Advanced Practice Provider to deliver direct clinical care to cancer patients in collaboration with our partner oncology practices. You will diagnose, evaluate, and manage cancer-adjacent comorbidities and coordinate care across primary and specialty providers to optimize health outcomes throughout the cancer journey.

Qualifications

• Current NP or PA certification required.
• At least 4 years of direct patient care experience in primary care and/or oncology.
• Extensive knowledge in chronic disease management and palliative care.
• Strong collaboration skills with diverse teams and positive work environment.
• Comfort with change and working in a start-up/growing organization.

Responsibilities

• Provide direct, evidence-based primary care to cancer patients, in-office and virtually.
• Maintain close collaboration with partner oncology providers managing the patient's cancer.
• Coordinate care and optimize referrals to primary and specialty care providers in the community.
• Participate in specialized clinical programs including CHF, frailty, and behavioral health.
• Provide transitions of care visits.
• Escalate clinical issues to the Medical Director when appropriate.
• Collaborate with VP of Clinical Operations to improve clinical operations.
• Aim for high scores on quality measures.
• Engage in interdisciplinary care team activities and case reviews.
• Perform other related responsibilities as needed.
