Qualifications

• Registered nurse with California license.
• Graduate of NLN accredited school of nursing.
• Current CPR certification.
• Two years in home health or public health preferred.

Responsibilities

• Complete initial patient and family assessments to determine home care needs.
• Develop and adjust care plans with nursing diagnoses and goals.
• Administer medications and treatments as prescribed.
• Provide health instructions and counsel patient and family.
• Coordinate with physicians and community resources to support care.
