The role includes initial assessments, care planning, medication administration, and patient/family education while coordinating with healthcare providers.

Eligible candidates must hold California RN licensure, CPR, and a valid driver’s license; experience in home health and IV therapy is preferred.

Qualifikationen

• California RN license required.
• 2+ years in home health or public health nursing preferred.
• Experience with IV therapy and wound care.
• Tracheostomy care experience.
• CPR certification required.
• Able to perform lab draws.
• Must have valid driver's license and insured vehicle.
• Graduate of NLN-accredited school of nursing.
• Self-directed with strong communication and organizational skills.

Aufgaben

• Complete initial patient and family assessment to determine home care needs.
• Re-evaluate nursing needs regularly and adjust the care plan.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs.
• Prepare clinical notes and communicate changes to the agency.
• Coordinate care with physicians and community health providers.
• Supervise and evaluate home health aides every two weeks.
• Ensure equipment and services are arranged.
