Twin Cities Physicians in Brainerd, MN is seeking a Wound Care Nurse Practitioner to provide on-site care across assisted living, memory care, and long-term care communities. You will manage complex wounds, support chronic disease management, and collaborate with physicians, nurses, and facility teams.

This full-time role offers a competitive total compensation of $120,000 to $160,000+ (base salary + productivity incentives) plus mileage reimbursement, full benefits, PTO, CME allowance,

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or related settings.
• Strong clinical judgment and ability to manage medically complex populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
