QuadMed seeks a Full-Time Wellness Coach for the HII Family Health Center in Newport News, VA. This on-site role follows a Monday–Friday schedule, focused on delivering health promotion activities and coaching to maximize participation and outcomes.

The successful candidate will coordinate programs, use EMR for documentation, and collaborate with internal and external resources to create comprehensive plans of care, all while supporting employee health goals and organizational wellness

Qualifications

• Bachelor’s Degree from an accredited college or university in health education, health promotion, nursing, nutrition, exercise science or another related field
• Minimum one (1) year of experience in the wellness industry, to include experience in the delivery and coordination of corporate based health promotions programs, one-on-one health coaching and/or personal training
• Current CPR (BLS) certification or must be obtained within 60 days of hire; CHES preferred; NBHWC-certified or within one year; Personal Trainer and Group Fitness certifications may be required within specified timeframes

Responsabilités

• Provide health outreach and wellness coaching via face-to-face, telephone, or telehealth to reduce high-risk behaviors and promote healthier lifestyles
• Document client activities and coaching session notes in the EMR
• Develop and implement assessment plans to determine population health and lifestyle needs
• Deliver population health risk management programs and collaborate with internal/external resources
• Engage employees and promote participation in Wellness and Health Improvement programs
• Lead wellness committee meetings and secure program buy-in across the organization
• Coordinate and deliver fitness and wellness programs including cardio, strength, mind/body, and fitness assessments
