Job summary

CVS Health is seeking an On-Site Wellness Coach to partner with employees at a New York client location. You will deliver one-on-one coaching, group programs, health screenings, and wellness initiatives to improve health outcomes and participation.

You will collaborate with leadership and wellness teams, using evidence-based behavior change techniques to drive measurable results while supporting the organization’s health and productivity goals.

Qualifications

3+ years of experience in wellness coaching or related field.
Experience coaching individuals and facilitating group programs.
Experience documenting interactions in EMR or wellness systems.
Proficiency with Microsoft Office.
Ability to travel daily to onsite client location.
Strong collaboration and communication skills.

Responsibilities

Provide individualized health and wellness coaching using evidence-based techniques.
Conduct health risk assessments and review biometric data to identify improvements.
Create customized wellness action plans aligned with goals.
Facilitate wellness workshops and group coaching sessions.
Track progress and report outcomes via EMR and wellness platforms.
