This full-time, on-site role emphasizes team leadership, on-call coverage, and coordinating with individuals, families, and care managers to meet personalized goals. You’ll ensure compliant documentation in the EMR, oversee resources and transportation, and advocate for meaningful, person-centered services while fostering strong partnerships with community

Qualifications

• High school diploma or GED required.
• Ability to communicate effectively with team and clients.
• Willingness to learn and adapt in caregiving environment.
• Valid driver’s license and reliable transportation.
• Ability to work in the U.S. without sponsorship.

Responsibilities

• Lead and maintain a positive team culture; participate in on-call duties.
• Oversee resources and operational needs, including scheduling and transportation.
• Coordinate with individuals, families and care managers to develop personalized plans.
• Ensure compliance and complete documentation in the EMR.
