Imagine the Possibilities, Inc. is seeking a Homes & Community Based Services Programmer to monitor services, develop strategies, and coordinate supports in an HCBS setting. You will lead a team and ensure goals are met with person-centered planning and thorough documentation.

The role is office-based, Monday–Friday, 8:00 a.m.–4:30 p.m., with on-call rotation. A high school diploma or GED, driver’s license, and reliable transportation are required; U.S. work authorization is a must.

Responsibilities

• Lead and develop a cohesive team culture with regular meetings and on-call coverage.
• Oversee resources such as vehicle use, budgets, and appointment scheduling to ensure smooth service delivery.
• Coordinate person-centered plans with CBCM and IDT; complete assessments and documentation.
• Serve as primary liaison with families, guardians, and case managers and promote community partnerships.
• Maintain compliance with laws and document incidents and services; advocate for independence and dignity.
