Job summary

Mecklenburg County is seeking a Re-Entry Services Case Manager to facilitate seamless transition of participants back into the community and promote employment stability.

This on-site role works with clients to provide comprehensive case management and access to services.You will develop individualized case/service plans, coordinate with partners, and assist with housing, identification, education, and treatment needs.

Qualifications

• Bachelor's degree in criminal justice, public administration, social work or related field.
• Minimum two years of directly related experience in case management.
• Experience working with felony and chronic misdemeanor offenders preferred.
• Ability to establish and maintain case plans and progress notes.

Responsibilities

• Provide professional level case management to ensure compliance with court orders.
• Develop and implement a unified case/service plan with clients and partners.
• Coordinate services and advocacy across multiple agencies.
• Educate and recruit housing providers and support providers through the vendor setup process.
