Job summary

Vantage Health Partners in Grand Junction, CO seeks a licensed Physical Therapist for a full-time, on-site role in a hospital/healthcare setting. The PT will evaluate, diagnose, and treat patients with musculoskeletal, neurological, and functional impairments, developing individualized plans and guiding patients through exercises to improve mobility and independence.

The position offers a competitive hourly pay range of $41-$46, a sign-on bonus, and opportunities for professional development

Qualifications

• DPT from an accredited program.
• Current state licensure to practice as a Physical Therapist.
• Strong knowledge of musculoskeletal and neurological rehabilitation.
• Proficiency in patient assessment, evaluation, and treatment planning.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced, multidisciplinary healthcare setting.
• Strong documentation skills and familiarity with electronic health records.
• Preferred: Certification in a specialty area such as orthopedics or sports rehabilitation.
• Preferred: Prior experience in an outpatient or hospital-based therapy setting.

Responsibilities

• Evaluate patients' physical condition, mobility, and functional abilities
• Develop and implement individualized treatment plans
• Perform therapeutic exercises, manual therapy, and rehabilitation techniques
• Educate patients and families on injury prevention and home exercise programs
• Document patient assessments, treatment progress, and outcomes accurately
• Collaborate with physicians and interdisciplinary care teams
• Adjust treatment plans based on patient response and recovery goals
• Maintain compliance with healthcare regulations and facility policies
• Recommend assistive devices and adaptive equipment when appropriate
• Support patient discharge planning and continuity of care
