Job summary

Vista Ridge Care and Rehab is seeking a compassionate Physical Therapist to join our in-house therapy team in Colorado. The role involves developing personalized treatment plans, guiding patients through exercises, and coordinating with a multidisciplinary team to improve mobility and quality of life for long-term residents.

The successful candidate will have active state licensure, experience in geriatric care, and the ability to supervise PT assistants.

Qualifications

• Colorado Physical Therapist licensure is required
• Geriatric care experience preferred
• Ability to supervise PT assistants and rehabilitation aides

Responsibilities

• Perform treatments and exercises to aid recovery and mobility.
• Develop individualized treatment plans based on assessments and referrals.
• Maintain and document patient progress and outcomes in the medical record.
• Provide discharge summaries per regulatory requirements.
• Communicate with patients, families, and health team members about plans and progress.
• Participate in trainings and maintain compliance with company policies.
