Job summary

Community Hospital in Grand Junction, CO is seeking a Physical Therapist for a PRN, on-site role. The incumbent will evaluate and treat patients across ages, implement and adjust plans of care, document outcomes, and participate in cost-related care planning.

The position requires a PT license in Colorado, a Bachelor’s degree (Doctorate preferred), and BLS certification within 90 days of hire. Relocation incentives and discretionary compensation may be available.

Qualifications

• Bachelor’s degree in physical therapy from an accredited program.
• Doctorate’s degree preferred.
• Current unrestricted license as a Physical Therapist in the State of Colorado (required).
• BLS Certification (must be obtained within 90 days of hire).

Responsibilities

• Coordinates and directs patient care services to patients of all ages to ensure that patient needs are met.
• Evaluates and assesses patients, establishes and implements a plan of care, and modifies treatment to achieve goals.
• Documents quality care in a timely manner per professional standards and regulatory guidelines.
• Collects and analyzes data from outcome measures to monitor progress and intervention benefits.
• Understands treatment costs and budgeting, staffing, billing/coding, and planning tasks.
