Job summary

Logan Health Medical Center in Kalispell, MT, seeks an RN Navigator to coordinate post-discharge care and collaborate with the orthopedic care team. You will schedule procedures, provide pre/postoperative education, and arrange follow-up visits and therapies to support patients through recovery.

Qualifications

• Active Montana RN license or compact license to practice in Montana.
• Bachelor’s degree in a related field and four years’ related experience, or equivalent.
• Six years’ related experience; four years in a specialty area (Orthopedics/post-op, OR, or similar).
• Knowledge of professional practice standards and regulatory requirements for specialty care.
• Proficiency with electronic health records and strong communication skills.

Responsibilities

• Coordinate follow-up care after ER visits or surgeries.
• Assist with surgery scheduling and patient education before/after procedures.
• Coordinate follow-up appointments, physical therapy referrals, and postoperative needs.
• Serve as a key connection between patients and the care team via in-person, online, and phone communication.
• Work on-site with the orthopedic/navigation teams to improve processes and outcomes.
• Support telehealth and remote monitoring tools to enhance care plans.
• Participate in process improvements and community education activities.
