Job summary

Logan Health Medical Center is seeking an experienced Registered Nurse to support our Urology Clinic in Kalispell, MT. You will educate patients on urologic conditions, coordinate appointments, and guide follow-up care across the care continuum.

The role emphasizes autonomy, strong communication, teamwork, and adherence to professional standards in a fast-paced clinic setting.

Qualifications

  • Montana RN License or multi-state compact license to practice in Montana is required.
  • Bachelor’s degree in a related field with four years’ experience or six years’ experience in a related area.
  • Four or more years’ recent specialty experience in one of the identified areas (Cardiology, Endocrinology, Gastro, Oncology, Orthopedics, Surgery, Pulmonology).
  • RQI Provider within 7 days of hire or transfer, or before independent patient care.

Responsibilities

  • Coordinate patient care across the continuum with the multidisciplinary team.
  • Educate patients about urologic conditions, procedures, and post-care requirements.
  • Assist with scheduling, follow-up care, and transitions from ED or inpatient settings.

Our Logan Health Specialty Navigation Team has an opportunity for an experienced RN to assist our Urology Clinic in Kalispell, MT!

What you’ll do:

In this on-site position at our Urology Clinic, you’ll support patients through in-person visits, phone calls, and online communication. You’ll provide education about urologic conditions, procedures, and ongoing care, helping patients understand their treatment plans and feel prepared for each step. You’ll coordinate appointments and procedures, support patients managing chronic urologic conditions, and arrange follow‑up care after emergency department visits. Through education before and after procedures, you’ll help patients follow care instructions, manage their recovery, and stay on track with follow‑up appointments and referrals.

We’re looking for a clinically sound RN with experience such inpatient , acute care, surgical, OR/Outpatient Surgery, Skilled Nursing, or other strong direct complex patient care experience.

Qualifications:

Current Montana RN License or a multi‑state compact license with authorization to practice nursing within the state of Montana; required.

  • Minimum of one (1) of the following required: Bachelor’s degree in a related field and four (4) years’ experience.
  • Minimum of six (6) years’ experience in a related area.
  • Minimum of four (4) years’ recent specialty experience in a minimum of one (1) of the identified specialty areas required: Cardiology Clinical Endocrinology Gastro Oncology Orthopedics Surgery Pulmonology Inpatient/Acute Care experience is preferred.
  • RQI Provider within 7 days of hire/transfer or before independent patient care, whichever comes first.
  • Knowledge of professional practice standards and regulatory requirements as related to specialty care area required.
  • Knowledge and experience with electronic health records required.
  • Must function with a high degree of autonomy, strong communication and interpersonal skills.
  • Possess the ability to develop and implement community education, outreach programs/workshops and seminars as defined for assigned specialty area required.
  • Possess ability to comprehensively assess patient’s psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s) required.
  • Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams required.
  • Promotes excellence through setting high standards and providing high quality outcomes.

Job Specific Duties:
  • Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.
  • Works closely with members of the multidisciplinary care team and their recommendations.
  • Exhibits professionalism and accuracy in interfacing with providers, clinic staff, patients, and family members involved in the plan of care.
  • Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable.
  • Assesses the physical, psychological, social and spiritual needs of the patient and family.
  • Identifies and assesses the learning needs of the population under care and/treatment.
  • Builds therapeutic and trusting professional relationships with patients, families, and caregivers.
  • Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).
  • Assists with the organization of the specialty interdisciplinary team meetings and facilitates the team’s recommendations in a professional and timely manner.
  • Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).
  • Works collaboratively with all stake holders to identify patient needs and assists in care and services, both medical and social, throughout the continuum.
  • Collaborates with the navigation team and leadership to optimize processes and metrics to reduce avoidable ED visits, inpatient admissions, hospital readmissions, cost of care, gaps in care, increase patient satisfaction, clinical outcomes, and provider satisfaction as defined for assigned specialty care area(s).
  • Assesses education needs and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.
  • Facilitates the use of telehealth and/or self‑monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.
  • Actively participates in activities that add value to organizational objectives and workflows, such as process improvement projects and/or community‑related initiatives to include community health fairs, workshops, etc.

Shift:

Day Shift - 8 Hours (United States of America) Schedule: Full‑time, 40 hours per week, Monday - Friday, 8-hour day shifts. Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
