Logan Health in Kalispell, MT, seeks a Nurse Navigator to coordinate postoperative care for orthopedic patients, assisting with scheduling and education before and after procedures. Qualifications

• Must hold Montana RN license or multi-state compact license to practice in MT.
• Bachelor’s degree in a related field with 4 years of related experience, or at least 6 years in a related area.
• 4+ years of recent specialty experience in Orthopedics, post-surgical care, or OR/pre/post operative care.
• RQI provider within 7 days of hire/transfer.
• Knowledge of professional practice standards and regulatory requirements.
• Experience with electronic health records.
• Ability to work with autonomy and strong communication.
• Ability to assess psychosocial status and patient needs.
• Ability to collaborate with Primary Care and multidisciplinary teams.
• Commitment to confidentiality and high ethical standards.
• Excellent verbal and written communication; professional diplomacy.
• Proficient with Microsoft Office and related software.

Responsibilities

• Coordinate post-acute care and follow-up after ER visits or surgeries.
• Assist with surgery scheduling and patient education before and after procedures.
• Coordinate follow-up appointments, physical therapy referrals, and postoperative resources.
• Serve as a key connector between patients and care team via in-person, online, and phone communication.
