Job summary

Avamere Transitional Care of Puget Sound in Tacoma, WA seeks a Licensed Nurse as MDS Coordinator. This full-time, on-site role coordinates patient care plans and reviews health status under state/federal rules.

You’ll work with the nursing team to ensure accurate assessments, documentation, and regulatory compliance. The ideal candidate will hold a nursing degree, an active WA RN license (OR license required per location), and have MDS/RAI experience.

Qualifications

• Nursing degree from an accredited college or university.
• Maintain current, unencumbered RN license in the state.
• Oregon: RN is required.
• Washington: RN is preferred.
• MDS certification preferred.
• Knowledge of RAI, CAAs, RUGs, and MDS 3.0.

Responsibilities

• Review resident care plans for goals and revisions.
• Ensure all care staff are aware of care plans.
• Schedule and facilitate care plan conferences.
• Monitor PPS and OBRA assessments.
• Ensure accuracy of clinical assessments and documentation.
• Comply with state/federal nursing guidelines and MDS processes.
• Oversee the MAR, flow sheets, and physician orders.
• Document care accurately reflecting resident response.
• Coordinate with other departments for consistent care.
• Participate in facility survey activities and report findings.
