Job summary

The MDS Coordinator at Avamere Transitional Care of Puget Sound in Tacoma, WA, serves as the primary contact for reviewing and coordinating resident care plans to promote quality care and services. This on-site, full-time position supports the clinical team and ensures that assessments and documentation meet state and federal standards.

Qualifications

• Nursing degree from an accredited college or university.
• Active RN license in WA (preferred in WA, required in OR).
• MDS certification preferred; knowledge of MDS 3.0, RAI, CAAs, and RUGs.

Responsibilities

• Review resident care plans for goals, problems, approaches, and revisions.
• Ensure all care staff are aware of care plans.
• Schedule and facilitate care plan conferences.
• Monitor PPS and OBRA assessments.
• Ensure accuracy of clinical assessments and documentation.
• Ensure compliance with state and federal guidelines including MDS processes.
• Oversee the UR process.
• Oversee MARs and physician orders; document care accurately.
• Coordinate with other departments to ensure consistency in resident care.
• Participate in facility surveys and report findings to management.
• Support Administrator and Director of Nursing.
