Job summary

Guardian Pharmacy, LLC is seeking a Long Term Care Pharmacist for a 100% on-site role in Cincinnati, OH. The position combines clinical expertise with operational oversight to deliver safe, effective medication care for long-term care patients.

The role requires a Pharmacy degree (BPharm or PharmD) and an active state license, with 1+ years of related experience. You will work in a fast-paced, team-oriented environment and participate in on-site 8:00 am–6:00 pm schedules, with additional on-call

Qualifications

• Bachelor's Degree (Pharmacy) or PharmD. required.
• State Pharmacist License required without restrictions.
• 1+ years of related experience.

Responsibilities

• Prepare medications for final dispensing, including filling, order entry, and packaging.
• Review orders and cycle-fill medications for accuracy.
• Verify legality and feasibility of dispensing based on allergies and interactions.
• Interpret prescription orders with professional judgment for patient safety.
• Adhere to Federal and State Board regulations and compounding guidelines.
• Oversee technicians and provide feedback to supervision.
• Manage inventory and coordinate ordering when below par.
