Job summary

Hamilton Health Box seeks a Licensed Practical Nurse to serve as On-Site Clinician (OSC) at our hybrid, telemedicine-enabled clinics. You will manage daily clinical tasks, greet employees, coordinate follow-ups, and deliver protocol-driven care under Providers’ orders.

You’ll work with care teams, pharmacies, and referral networks to ensure high-quality, efficient care and health education. Travel between local clinics may be required.

Qualifications

• LPN/LVN license is required.
• Current BLS certification required.
• Ability to work on-site and travel between clinics as needed.

Responsibilities

• Carry out daily clinical and administrative operations at the HHB site.
• Schedule follow-ups, perform screenings, and document results.
• Coordinate care with providers, pharmacy, and referrals.
• Provide one-on-one counseling and health education to employees.
• Maintain compliant records in EMR and support program initiatives.
• Travel between local clinics as required.
