Job summary

Hamilton Health Box is seeking an On-Site Clinician (OSC), a Licensed Practical Nurse (LPN/LVN) who will deliver protocol-driven care within our on-site clinics. The OSC will coordinate with providers, schedule and screen employees, and participate in health education and preventive programs.

The role includes travel to local clinics as needed and maintaining EMR records, contributing to a high-touch, low-cost patient experience for our clients and their staff.

Qualifications

LPN/LVN with active Texas license.
BLS certification required before onboarding.
2+ years healthcare experience preferred.

Responsibilities

Collaborate with departments to ensure employee care and programs.
Develop individualized health programs from risk assessments.
Assist with physicals, screenings, and follow-up exams.
Schedule follow-ups and return-to-work exams.
Coordinate referrals and document progress in medical records.
Lead health education sessions and health fairs.
Document utilization and outcomes in records.
Provide counseling and interpret screening results.
