Job summary

Hamilton Health Box (HHB) is hiring an On-Site Clinician (OSC) to lead daily clinical and administrative operations at HHB locations. You will interact with employees for scheduling and follow up, deliver protocol-driven care, and implement preventative health initiatives.

The OSC coordinates care with providers and partners, supports health education, and builds strong relationships with staff, patients, and the community. Some travel to local clinics may be required.

Qualifications

• Licensed Practical Nurse with current license.
• BLS certification required.
• Ability to communicate clearly with staff and patients.

Responsibilities

• Carry out daily clinical and administrative operations at HHB location.
• Interact with employees for scheduling, follow-up, and care delivery.
• Deliver protocol-driven care as ordered by the Provider.
• Coordinate referrals and health education initiatives.
• Train and develop skills; maintain relationships with staff, patients, and community.
• Document visits and education in EMR; may travel between clinics.
