Job summary

Sarah's Circle is hiring a Case Manager (Interim Housing) to provide intensive case management for clients seeking housing and stability in Chicago. The role is on-site and non-exempt, with a typical 9:00AM-5:00PM schedule and occasional evening meetings.

Responsibilities include intake, assessments, service planning, and collaboration with landlords. A bachelor’s degree in a related field is preferred, with bilingual English/Spanish a plus and opportunities for LCSW certification supervision.

Qualifications

• Bachelor’s degree in psychology, social work, or related social service field preferred.
• Experience working with individuals experiencing homelessness and/or trauma required.
• Bilingual English/Spanish preferred.
• Previous case management experience in homeless services preferred.

Responsibilities

• Provide case management services to help clients obtain permanent housing and stabilize income.
• Complete intake, assessments, and service plans in a timely manner.
• Develop goal-based plans with clients and conduct regular progress meetings.
• Offer coaching, life skills guidance, and referrals to needed services.
• Identify barriers such as substance use or mental health to support goals.
• Assist clients with housing search, leases, and landlord coordination.
• Support moving, furniture, and essential home needs as needed.
• Conduct Coordinated Entry Assessments as required by program.
