Sarah’s Circle in Chicago seeks a dedicated Case Manager (Interim Housing) to provide intensive case management for women seeking housing. You will coordinate services, document in HMIS, and collaborate with partners to ensure housing stability.

On-site, full-time role with schedule Mon–Fri, occasional evenings. The position requires a bachelor’s degree in a related field, bilingual English/Spanish preferred, and ability to travel locally.

Qualifications

• Bachelor’s degree in psychology, social work, or related field preferred.
• Bilingual English/Spanish preferred.

Responsabilités

• Provide case management to support clients in obtaining permanent housing and improving income and stability.
• Complete intake, assessments, service plans, and progress reviews; enroll clients timely.
• Develop individual goal-based service plans and conduct regular progress meetings.
• Provide coaching, resources, and referrals to help clients achieve goals and access services.
• Identify and address short-term or chronic issues affecting progress, including mental health or substance use.
• Assist clients with housing searches, lease facilitation, and landlord relations.
• Support with moving, furniture, and home essentials as needed.
• Conduct Coordinated Entry Assessments and participate in Housing Resource Specialist meetings.
