Qualifications

• Bachelor’s degree in human services, social work, psychology or related field required.
• Certified Peer Specialist certification required.
• Certified Psychiatric Rehabilitation Practitioner certification required within 12 months of hire.

Responsibilities

• Meet with referred individuals to assess housing needs and develop stability plans.
• Maintain documentation for move-in, recertification, and ongoing case records.
• Conduct regular home visits and support during transitions.
• Coordinate transportation for appointments and interviews to promote stability.
• Collaborate with landlords and community resources to address barriers to housing stability.
• Develop IRPs and strategies with the individual to achieve personal goals.
• Communicate with supervisors and participate in weekly supervision.
