Job summary

Access Services seeks a compassionate Housing Case Manager in Fort Washington, PA to empower individuals across Montgomery County on the journey toward stability and wellness. The role blends mental health and housing stability skills in a full-time, on-site setting.

You will develop recovery-oriented IRPs, coordinate with partners, and advocate for clients while meeting HUD guidelines and documentation needs.

Qualifications

• Bachelor's degree in a related field including human services, social work, psychology or special education required.
• Certified Peer Specialist certification is required.
• Certification as Psychiatric Rehabilitation Practitioner within 12 months is required.

Responsibilities

• Meet with referred individuals with Housing Locator to support housing placement.
• Develop and update Housing Stability Plans every three months.
• Maintain documentation for move-in, recertification and ongoing needs.
• Conduct monthly home visits and support during transitions.
• Coordinate transportation for health appointments and job activities.
• Identify barriers to housing stability and propose solutions.
• Connect clients to social, health, and benefit resources.
• Ensure HUD compliance and funding guidelines.
• Attend meetings and case reviews with the team and external agencies.
