Access Services seeks a dedicated Housing Case Manager in Fort Washington, PA to support housing stability for individuals in Montgomery County. The role blends mental health and housing assistance, coordinating with landlords and local resources to secure stable housing.

You will develop Individual Rehabilitation Plans, connect clients with social and health services, and ensure HUD compliance. A strong, person-centered approach and crisis-resilience are essential.

Qualifications

• Bachelor’s degree in human services, social work, psychology or related field
• Certified Peer Specialist certification
• Certification as Psychiatric Rehabilitation Practitioner within 12 months of hire
• Some experience in mental health services
• Experience in supportive housing systems preferred
• Strong interpersonal and communication skills

Responsabilités

• Meet with referred individuals with the Housing Locator to support housing transitions
• Develop and update Housing Stability Plans with quarterly updates
• Maintain organized documentation for move-in, recertification, and ongoing records
• Conduct monthly home visits and provide transition support
• Assist with transportation for health appointments and job interviews
• Identify barriers to housing stability and provide creative solutions
• Connect individuals to social, health, and benefit resources
• Ensure HUD compliance and funding guidelines
• Participate in meetings and case reviews
• Develop and regularly update IRPs for Mobile Psychiatric Rehabilitation
• Maintain accounting for disbursement and funds for operations and needs
