Friendship Place is hiring a Case Manager in Washington, DC to provide primary service coordination and non-congregate shelter support to residents experiencing homelessness. You will manage a caseload of 25, conduct intakes, coordinate housing and health services, and document outcomes per contract requirements.

The role requires a high school diploma or GED with relevant experience, trauma-informed practice knowledge, and the ability to work on-site.

Formación

• High School Diploma or GED with 2 years of experience working with individuals or families experiencing homelessness, behavioral health challenges, or related barriers.
• Knowledge of trauma-informed care, harm reduction, motivational interviewing, and Housing First principles.
• Must have or be able to obtain a First Aid/CPR/AED certification that includes hands-on skills testing; Friendship Place will provide training.
• Proficiency in Microsoft Word and data management systems; HMIS experience preferred.
• Bilingual (Spanish/English) a plus.

Responsabilidades

• Provide Housing First, client-centered, goal-oriented case management to assigned clients with a goal of exiting to stable housing.
• Maintain a caseload of at least 25 residents.
• Conduct intakes, assessments, and referrals of eligible residents per contract guidelines.
• Collect all necessary resident documentation to support service access and housing outcomes.
• Document case management services, case plans, housing information, exit plans, assessments, and other reports with detail and timeliness.
• Engage with community services/agencies, service providers, and key stakeholders for coordination of care.
