Job summary

Friendship Place in Washington, DC is seeking a Case Manager – Neighbors First Families to provide housing-focused services to families experiencing homelessness. The role emphasizes wraparound supports, strong field engagement, and collaboration with stakeholders to secure housing resources.

You will conduct needs assessments, develop service plans, document outcomes, and participate in supervision while upholding the organization’s core values and trauma-informed approach.

Qualifications

• HS Diploma/GED with 6 years of direct human service experience or
• Bachelor’s degree with 2 years of experience in counseling/case management or
• Master’s degree in social work or related field with 1–2 years of experience required.

Responsibilities

• Provide supportive services to individuals experiencing homelessness with serious mental illnesses, addictions, or medical concerns.
• Deliver face-to-face contact with clients, spending about 80% of time in the field.
• Develop, implement, and revise individual service plans and biopsychosocial assessments.
• Assist households in locating and securing wraparound services and housing resources.
• Document participant engagement and outcomes in the OCTO/HTH system within 48 hours.
