Haymarket Center is seeking a Quality Improvement Coach to partner with clinical and operational leaders in order to enhance patient outcomes and strengthen clinical workflows. The role focuses on improving documentation quality, regulatory readiness, and the overall quality of care across the continuum of care.

Using data analysis, process improvement methodologies, and change management, you will translate standards into practical processes and serve as a coach and educator to frontline staff,

Competenze

• CADC or licensed by IDFPR with SUD experience.
• CCE of CHC (Preferred).
• At least 2 years of record keeping experience with a substance abuse treatment facility.
• Familiarity with Administrative Code Part 132, 140, 2030, 2060, 2090, and CARF Behavioral Health Standards.
• Familiar with Health Center Program Standards by HRSA and SAMHSA/State of Illinois standards.

• Conduct reviews of clinical records to evaluate compliance with treatment standards and policies.
• Review treatment and recovery plans to ensure documentation of needs, goals, interventions, and progress.
• Monitor clinical documentation to assess patient progress toward treatment objectives.
• Validate medical necessity and continuing stay criteria.
• Review discharge documentation and continuing care plans for continuity of care.
• Document audit findings and identify opportunities for quality improvement.
• Translate audit findings and outcome measures into meaningful improvement projects.
• Collaborate to develop action plans addressing identified gaps and sustain improvements.
• Use PDSA cycles, root cause analysis, and process mapping in quality initiatives.
• Follow-up reviews to measure progress and effectiveness of actions.
