Job summary

Spectrum of Solutions in Midland, TX seeks a Case Manager to coordinate programs and support services for individuals with IDD. This non-exempt, on-site role reports to the Executive Program Director and requires a high school diploma and travel as needed.

Qualifications

• High school diploma/equivalent required.
• 1–2 years of experience in an equivalent role preferred.
• Must be able to work in an office (not a remote role) and travel for company business as needed.
• Valid Texas driver’s license and related insurance.
• Knowledge of inventory and supply ordering, distribution, and control processes.

Responsibilities

• Coordinate program plans by leading meetings and documenting goals and ensure annual review of plans.
• Deliver program plans by placing training documents in service locations and training staff on implementation and data collection.
• Monitor goals by overseeing staff interactions and data collection.
• Integrate services by finding community services that accept Medicaid.
• Record progress by directing staff in data collection and reviewing, and adjusting goals based on progress.
• Communicate with families and agencies.
• Develop pre-discharge and pre-admission plans.
• Ensure compliance with guidelines.
