Job summary

NewYork-Presbyterian Hospital is seeking a Care/Program Coordinator for the Comprehensive Health Program, grant-funded, to support frontline providers and care coordination staff in engaging and retaining clients.

You will assist with referrals, access to Social Determinants of Health resources, and coordinate between clients, groups, and program staff to ensure program goals are met.

Qualifications

• Bachelor's Degree or minimum of three (3) years of equivalent work experience.
• Excellent engagement skills.
• Strong interpersonal skills, highly organized, ability to work with diverse community groups and multi-task.
• Master's degree preferred.
• Bilingual in Spanish preferred.

Responsibilities

• Provide client and programmatic support, including accompaniment and navigation.
• Facilitate initial engagement, retention, and reengagement of clients.
• Assist with referrals and access to Social Determinants of Health services.
• Coordinate between clients, groups, and program staff to meet goals.
