Postgraduate-Center-For-Mental-Heal is seeking a Care Coordinator to join the Care Coordination team in New York. Under the Program Supervisor, you will address member needs, update care plans, and perform outreach between visits to support wellness and reduce ER visits.

You will coordinate with primary and behavioral health providers, assist with transportation and appointments, maintain documentation, and advocate for members. This role requires CASAC and 4+ years of related experience.

Qualifikationen

• CASAC certification with 4+ years of related human services experience.
• Experience with chronic health and/or behavioral health disorders.
• Strong documentation and communication skills.

Aufgaben

• Coordinate care for a caseload of 40-50 members.
• Maintain monthly contact with all members and engage newly enrolled/high-risk members.
• Conduct needs assessments and assist with care plan development and execution.
• Advocate for members and coordinate with providers and community resources.
• Manage records, reports, and ensure timely documentation; provide crisis intervention as needed.
