Griswold Home Care for Tulsa is seeking a Care Coordinator to match clients with caregivers and manage ongoing needs on-site in Tulsa, OK. You will act as a case manager, ensuring the best care and reliable communication for families and caregivers.

Ideal candidates bring an associate degree in human services or related field, plus experience in home care or a strong desire to help others. Weekend/evening hours may be required.

Qualificações

• Associate degree in human services or related field is preferred but not required.
• Experience in home care, human services, or a similar field is required (or a strong desire to help others).
• Must pass Griswold hiring criteria per company standards and/or state regulations.
• Reliable transportation, valid driver’s license, and car insurance.
• Must operate a laptop, mobile phone, and related apps.
• Ability to see and interpret information in systems and record data accurately.
• Must lift up to 40 pounds and alternate between stationary and walking/standing.
• Weekend and/or evening hours may be required.

Responsabilidades

• Build relationships with clients and families to match with caregivers.
• Match caregivers to clients to fit needs and improve retention.
• Complete monthly check-ins with clients (text, email, phone, in-person).
• Keep scheduling system up to date with client schedules and preferences.
• Participate in on-call duties on a rotating basis.
• Collaborate with Talent Specialist to ensure a positive ongoing experience for clients and caregivers.
• Support onboarding for new clients to ensure a thorough, welcoming process.
