Ennoble Care is seeking an Embedded Care Coordinator to join our team in the Leesburg area, working on-site within partnered long-term care facilities. You will serve as a trusted member of our clinical team, building relationships with residents and families and supporting care coordination and remote patient monitoring.

The role emphasizes a family-practice style within a long-term care setting, requiring strong communication, organization, and a proactive, patient-centered approach.

Qualifications

• Legally authorized to work in the United States.
• Minimum 1 year of patient-facing experience in a long-term care or similar setting.
• Proficient with Microsoft 365 apps (Outlook, Word, Excel).

Responsibilities

• Coordinate care for long-term care residents using the EMR.
• Maintain regular on-site engagement with residents to support care plans.
• Collaborate with Ennoble Care providers in the long-term care setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Communicate patient needs and changes to the clinical team promptly.
• Document patient interactions and care coordination activities.
• Follow established workflows and escalation protocols.
• Contribute to team collaboration and facility partnerships.
