Griswold Home Care for Howard County, Maryland is seeking a full-time Care Coordinator to manage client intake, caregiver scheduling, and client-family relations for our growing home care agency serving older adults.

The role is on-site in Howard County with Monday–Friday, 8:00 AM–4:00 PM and occasional on-call coverage. You will ensure great matches and experiences by coordinating care teams and maintaining compliant client files.

Qualifications

• Experience in care coordination or scheduling roles.
• Ability to build relationships with clients and families.
• Familiarity with home-care regulatory requirements.

Responsibilities

• Build meaningful relationships with clients and families to match with caregivers.
• Develop relationships with caregiver team and assign based on skills and needs.
• Conduct weekly check-ins to ensure client satisfaction.
• Coordinate onboarding of new clients joining Griswold.
• Follow up on digital leads using established processes.
• Maintain compliant client files per Maryland Department of Health.
