Job summary

Circadia Health is seeking a Nurse Practitioner to join as an on-site, full-time team member in Pasadena, TX. You will perform initial and follow-up assessments on residents flagged by Circadia signals and work to integrate AI-driven insights into daily care workflows.

You will train facility staff, manage care plans, and document thoroughly. A Texas NP license, independence in practice, and 2+ years of experience with complex adults are required, with travel within the local area welcomed.

Qualifications

• Must have Texas Nurse Practitioner license and board certification where applicable.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; ability to practice independently.

Responsibilities

• Conduct initial and follow-up assessments for flagged residents.
• Train and coach facility staff on Circadia integration into workflows.
• Interpret trend-based insights to drive timely interventions.
• Complete documentation within the same day.
• Perform high-volume visits across designated SNFs (30+ per day as needed).
• Provide feedback to improve workflows and tech integration.
