Job summary

Circadia Health is seeking an on-site Nurse Practitioner to join our care team at a partner facility in Lancaster, CA. You will conduct initial and follow-up assessments, translate AI-driven insights into direct patient care, and train facility staff to weave Circadia into daily workflows.

This full-time role emphasizes hands-on patient care, rapid documentation, and collaboration with clinical ops and product teams to reduce readmissions.

Qualifications

• Certified and licensed NP in MI; ANCC certification in Adult/Family/Geriatric/Acute care.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MI driver's license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment; English required.

Responsibilities

• Direct patient care: assess residents and plan care.
• Tech → bedside integration: train staff and integrate Circadia into clinical workflows.
• Interpret trend-based insights and prioritize high-risk patients for timely interventions.
• Complete documentation/charts within the same day.
• High-volume rounding across SNFs (typ. 25+/day) focusing on new admits and Circadia alerts.
• Mentor NP onboarding and collaborate with clinical ops/product teams.
