Job summary

Ascend Health is seeking a compassionate Registered Nurse (RN) for a part-time on-call role in Northern Utah (Logan area). You will perform after-hours nursing visits, triage, and family support within Home Health & Hospice, collaborating with the interdisciplinary team to ensure timely response and continuity of care.

The position requires current Utah RN licensure, at least 1 year of nursing experience (Home Health or Hospice preferred), strong clinical assessment, excellent communication, and

Qualifications

• Requires current Utah RN license.
• Minimum of 1 year nursing experience, preferably in Home Health or Hospice.
• Strong clinical assessment and communication skills desirable.
• Reliable transportation and a valid driver’s license required.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide skilled nursing visits and triage after hours.
• Respond to calls for triage and support in a timely manner.
• Communicate effectively with physicians and oncoming staff.
• Maintain accurate documentation in the EMR.
• Deliver compassionate care aligned with Ascend Health's mission.
