Job summary

Heart to Heart Hospice is seeking a compassionate Registered Nurse on call to support patients and families in the community. You will perform home visits for assessment, treatment, and crisis management, including pronouncement of death when needed, and develop nursing care plans with the patient’s physician and IDG.

The role requires graduation from an accredited nursing program, an active State RN license, a valid driver's license, and reliable transportation.

Qualifications

• Must be a graduate of an accredited nursing program.
• Current State RN License.
• Two years of RN experience preferred in a healthcare/clinical setting.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including pronouncement of patient death when applicable.
• Develop and revise nursing care plans with the patient’s physician and IDG.
• Maintain excellent customer care skills with internal and external customers; use proper phone etiquette.
• Prepare clinical/progress notes demonstrating progress toward goals.
• Inform physician and other personnel of changes in patient needs and outcomes; evaluate patient/family response to care.
• Timely submission of all documentation.
• Perform other duties as required; ensure compliance with laws and Medicare regulations.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established policies.
