Job summary

Xencare Inc. (DBA Wellpointe Hospice) seeks an RN for on-call nights and weekends at Fresno Guest Homes Facilities. The role focuses on delivering compassionate hospice nursing outside regular hours to ensure continuity of care.

Ideal candidates hold a California RN license, with hospice experience preferred and CHPN a plus. Strong assessment, EMR proficiency, and willingness to travel are essential; bilingual skills are a bonus. Competitive compensation and benefits offered.

Qualifications

• Active RN license in California.
• Hospice or palliative care experience preferred.
• CHPN certification preferred but not required.
• Ability to make autonomous clinical decisions in the field.
• Strong empathy and communication with patients and families.
• Reliable transportation and valid driver’s license; travel required.
• Familiar with electronic documentation and EMR systems.
• Bilingual skills are a plus.

Responsibilities

• Respond to patient calls, triage needs, and deliver hospice care.
• Assess and manage pain, SOB, agitation, and symptoms.
• Provide skilled nursing care per plan of care and hospice protocols.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary hospice team for coordinated care.
• Support families with education and emotional guidance.
• Document interactions, assessments, and changes in condition.
• Travel to patient residences for in-person care when needed.
• Participate in team meetings and ongoing staff development.
