Job summary

Wellpointe Home Health is seeking a dedicated Registered Nurse Case Manager to deliver critical home health nursing care during non-business hours in Fresno, CA. You will respond to urgent patient needs overnight, on weekends, and holidays, ensuring continuity and quality of care.

This role suits an experienced Home Health nurse who can work independently, provide compassionate patient support, and collaborate with the Hospice Operations team to maintain high standards of care in the Fresno

Qualifications

• Active RN license in California.
• 1 year of Home Health or palliative care preferred.
• Strong clinical assessment skills with autonomous field decisions.
• Empathy and communication with patients and families in difficult moments.
• Valid driver’s license, proof of auto insurance, travel required.
• Experience with electronic medical records (EMR).
• Bilingual in Spanish, Hmong, or other relevant languages is a plus.

Responsibilities

• Triage patient calls and deliver timely Home Health care.
• Assess and manage symptoms (pain, dyspnea, agitation).
• Provide skilled nursing per plan of care and Home Health protocols.
• Administer medications and perform prescribed treatments.
• Collaborate with the interdisciplinary Home Health team for coordinated care.
• Educate and support families and caregivers.
• Document all interactions, assessments, and changes in condition.
• Travel to patient residences to deliver in-person care when needed.
• Attend team meetings and participate in ongoing staff development.
