Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid drivers license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload including the assessment,Determining eligibility, admission, processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess information from patient and family factors that may impact needs and initiate intervention by other team members.
• Initiate communication with the attending physician and interdisciplinary team for coordination of care.
• Provide regular visits to reassess the patient's needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans Timely.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What Youll Do
