Westminster Communities of Bradenton is seeking an On-Call Physical Therapist to provide patient assessments, care planning, direct treatment, and clinical collaboration in the Health Center. You may supervise licensed therapists, assistants, technicians, aides and students as required, with on-call scheduling as needed.

Responsibilities include evaluating conditions, creating care plans, documenting progress, and participating in program development while supporting care giver staff and staying

Competenze

• Must be licensed as a Florida Physical Therapist.
• One year of long-term care experience preferred.
• CPR certification required.

Mansioni

• Evaluate and treat physical conditions to restore function and mobility as ordered by MD.
• Conduct assessments to determine the need for therapy and provide treatment modalities.
• Maintain timely, accurate documentation and update the system.
• Perform Resident Home Assessments as part of discharge planning.
• Screen residents during admission and periodic re-evaluations to determine therapy needs.
