Job summary

Westminster Communities of Bradenton is seeking an On-Call Physical Therapist to join the Health Center team. The role involves resident assessments, care planning, direct treatment, and staff training across a general rehab caseload, with supervision of licensed therapists and aides.

Schedule: On-Call, as needed. Florida license is required; prior long-term care, med-surg, or home health experience is advantageous.

Qualifications

• Must be currently licensed in the State of Florida as a Physical Therapist.
• One year of long term care desirable.
• Med-surg, Home Health experience helpful.
• Experience with handicapped, developmentally disabled, or older adults preferred.

Responsibilities

• Evaluate and treat physical conditions to restore function and mobility; design and carry out care plans.
• Conduct assessments to determine therapy needs using modalities such as electrical stimulation and therapeutic massage.
• Maintain timely, accurate documentation in the system.
• Perform Resident Home Assessments as part of discharge planning.
• Screen residents at admission and conduct quarterly and annual reviews for therapy needs.
