Volunteers of America Chesapeake & Carolinas seeks an experienced LPN Health Services Coordinator in the District of Columbia. The role involves nursing consultations, care planning, and collaboration with care teams to schedule treatments and follow-ups.

The ideal candidate educates patients about conditions and medications, develops care plans, and ensures compliance with healthcare laws and organizational guidelines while maintaining compassionate, patient-centered care.

Qualifications

• Be a graduate of an accredited nursing school with registration and license (LPN).
• Ability to read, write, and speak English clearly.
• Experience in residential/homeless health settings is a plus.
• Bilingual in any language is a plus.

Responsabilités

• Be available, on-call, as scheduled, for nursing consultation and treatment of minor illnesses or injuries.
• Develop rapport with each person receiving nursing care.
• Participate in the pre-admission evaluation, generation plan of care, Physician order sheets, MAR, and all documentation-related nursing processes.
• Prepare Medication History at admission and perform periodic re-evaluation and reviews.
• Coordinate medical appointments, transportation, and follow-up.
• Provide staff instruction on medical care and operate with House Monitors.
• Assess, plan, and follow up on medical issues noted with people.
